In this blog, I’m going to show you how to add sign-up forms to your Shopify store that connect to your Klaviyo account so you can grow your list and get more sales with email marketing. The video below goes through this with a screen recording, so I recommend checking that out too.
Sign-up forms on your website and Pop-Ups are central to regular organic list growth. They ensure website visitors are prompted to join your mailing list, giving you permission to email them, and allowing you to nurture them to become customers.
There are two types of forms you need to add. The first is a Pop-Up, which is the most effective way to collect email addresses. The second is the sign-up forms that are embedded on your website, most commonly in the footer and some landing pages.
These instructions are for Shopify Plus and Shopify specifically, but it’s a similar process for other platforms.
Before you add any forms to your site, you need to add the Klaviyo Signup Form Code Snippet.
When setting this up please make sure you have your Klaviyo page, your ecommerce platform (in this case Shopify or Shopify Plus) and your store site opened.
In the Klaviyo dashboard, go to your Signup Forms page from the left panel, or if you’re logged in, go to www.klaviyo.com/forms
Before we build the popups or forms, you will need a List within Klaviyo to collect the subscribers, we recommend triggering a welcome flow from the list to nurture subscribers.

Begin with the background image, change the image to something relevant to your brand:
You can also customise the email input block and button to be more aligned with the brand colours or background image.
Finally, choose the rules you would like to use. We recommend:
a. “Show after scrolling 30% of the page” As we would like to fully showcase the business and then give customers an opportunity to get a discount.
b. Display Frequency → Don’t show again after submitting the high-quality revenue-generating, form or go to URL action.
i. Show again 5 days after closing
c. Have the Pop-Up available on Desktop and Mobile (unless you want a separate design for each).
d. Don’t show Pop-Up to existing Klaviyo profiles.
e. Don’t show Pop-Up on certain URLs.
i. Cart pages / Checkout pages
ii. Contact us
iii. Giveaway landing page.
Make sure to check your design before activating it. To activate, change the status from “Draft” to “Live” and Publish. Refresh or go to your store site, your Pop-Up should now be visible. If your list is linked to a flow (E.g. Welcome Series), you can test it by putting in your email address.
Now when a visitor fills in the Pop-Up, their profile will be created on Klaviyo.
Embedded forms are the forms around your site, including your footer, asking for users to subscribe. It’s important to connect your website embedded forms to your Klaviyo account, or replace them with Klaviyo forms, to ensure that all forms on the site are properly collecting subscribers. Most Shopify themes automatically connect Shopify and Klaviyo when setting up the initial integration, but if they don’t then this step can be tricky.
In the Klaviyo dashboard, go to your Signup Forms page from the left panel, or if you’re logged in, go to www.klaviyo.com/forms.
Now your forms and pop-ups are ready to collect your visitors’ emails! Make sure you link the Pop-Up / Embed Form list to your Welcome Series Flow so you can nurture your subscribers. Make sure you always send quality content and newsletter campaigns. If you do this, you are all set to generate a lucrative stream of revenue.
